Angiographic predictors of adverse outcomes in the modern interventional era.

چکیده

The development of preprocedural angiographic risk stratification criteria was meaningful in the balloon angioplasty era (1). Acute ischemic complications consisted of angiographic complications with impairment of epicardial coronary blood flow. The demonstration that these postpercutaneous coronary intervention (PCI) ischemic complications were related to the preprocedural angiographic complexity of the lesion allowed operators to riskstratify patients. Current practice strategies have made non-angiographic complications, reflected by rise in creatine kinase MB fraction (CK-MB) enzyme, the most common post-PCI adverse event. The clinical impact of these rises in cardiac enzyme after PCI has been debated, but current evidence supports their negative prognostic value, particularly for larger increases in CK-MB (2).

Background: Percutaneous Coronary Intervention (PCI) for bifurcated lesions is associated with a low rate of success and high rate of complications compared to such treatment of lesions of most other morphologies. Symptoms occurring at the time of procedure caused by PCI may require additional angiography with or without stenting through stent struts, which subsequently increases risks of reste...

Management and outcomes of coronary artery perforation during percutaneous coronary intervention.

Coronary perforation is a particularly feared complication of percutaneous coronary intervention. The optimal management and predictors of adverse outcomes for these patients remain to be defined. Advances in management such as the use of polytetrafluoroethylene-covered stents have not been critically examined in terms of efficacy. We analyzed a cohort of patients who sustained coronary perfora...

Clinical outcomes in a community-based single operator coronary interventional program.

BACKGROUND Physicians who perform fewer than 75 coronary interventional procedures annually and centers where fewer that 200 procedures are performed annually are considered to be low-volume and likely to have higher procedural risk and suboptimal outcomes. METHODS This was assessed in a retrospective analysis of clinical outcomes in 559 patients who underwent multi-device coronary interventi...

Infrapopliteal application of paclitaxel-eluting stents for critical limb ischemia: midterm angiographic and clinical results.

PURPOSE To report the midterm (<or=1 year) angiographic and clinical outcomes of a prospective study investigating the infrapopliteal application of paclitaxel-eluting stents (PES) in patients with critical limb ischemia (CLI). MATERIALS AND METHODS Infrapopliteal angioplasty was chosen as first-line therapy in patients with unilateral or bilateral CLI and additional femoropopliteal angioplas...

A comparative study of rotational atherectomy in acute and stable coronary syndromes in the modern era.

Percutaneous rotational coronary atherectomy (PRCA) is commonly used in the percutaneous treatment of diffuse, calcified coronary lesions in stable coronary syndromes (SCSs) and facilitates successful delivery and deployment of balloons and stents. Early experience with PRCA cautioned its use in acute coronary syndromes (ACSs). However, the evolution of the PRCA technique and improved antiplate...
